WHO IS ELIGIBLE:
-Pregnant, Breastfeeding, and Postpartum Women
-Infants and Children under 5 years of age
HOW TO APPLY:
-Must meet income guidelines below or;
-Receive Medicaid, Food Stamp, Ohio Works First (TANF), Refugee
Resettlement or Disability Assistance from Job and Family Services
-Call or stop by the clinic to make an appointment
